currently describing the the state transition when releasing a held appt as going from a status of pending to cancelled.

This makes sense for the "Pre Fetch" use case. But for the real time use cases one could imaging a going from pending back to proposed. Essentially allowing the end used to change his/her mind and hold and release and hold again.
