Greetings friends. My institution is doing some foundational work with the WHO in mapping their clinical guidelines to standard terminologies and FHIR to enable computable guidelines. We are in the midst of some early pilot demonstration work with them, and we have decided to start from either the IPS or USCDI as an intermediate EHR patient object representation. As it relates to USCDI/Argonaut, we found it difficult to figure out where we'd graft in pregnancy history. Any immediate thoughts come to mind? Happy to give more context if necessary.

Pregnancy history would principally be a collection of Observations.

@Danielle Friend @Jenni Syed how is this appearing in your servers?

Public health has already worked on this in the ECR guide... @Sarah Gaunt can provide more details. Whatever we do needs to align with their needs as well.

well, but is that what Argonaut servers are actually doing?

You can ignore public health needs but eventually will have to deal with it sooner or later this stuff gets written into regulations too typically following a population health scare...

Hmm, now this is something that didn't occur to me (using an electronic case report as the intermediate EHR form)

That seems like the default backstop option to any circumstance where previous groups haven't rendered an opinion. :) Everything can be a bag of observations, right? :)

The data in a pregnancy record is primarily observations. There's a wee bit of medications and procedure stuff in there, but all of the measurements, reports of last menstrual period, past pregnancies, etc. are all point-in-time observations.

The grouper for everything would be EpisodeOfCare

Thanks @Lloyd McKenzie ... hope you're well, btw! :)
