Healthedata1 opened Issue #26

{indexed](http://hl7.org/fhir/documentreference-definitions.html#DocumentReference.indexed) is for some reason 1..1 in the STU3 core... per our profile design convention we need to add to the differential.

yes indexed in STU3 was mandatory, holding the creation time for the documentReference. Current has relaxed and clarified this. Thu MHD used this to hold the DocumentEntry.submissionTime as per the mapping shown... Note MHD has full structureDefinitions published now in Simplifier https://simplifier.net/ihepatientdemographi?fhirVersion=STU3
