It was defined in my build when we authored it but looks like it never got published. I'll add an issue. Also there is no link to your "here"...

I don't know the answer because I have always strictly adhered to the same version for the code system and the valueset when defined in FHIR. USCore codesystems start at STU3 - Based on your question - It sounds like that is not a problem...

Here is the original codesystem in the spec. In DSTU2 there was this really sweet way to define the URI so I bunched all the codes into a single codesystem
