How are they represented in Argonaut / US Core? I would sort of expect them in DiagnosticReport - but its profile says only LAB.

Alcohol, Pregnancy is not covered by US Core. If the US implementers can agree on something and test and pilot it then it would be great to add. ( or US regulators specify something) ... Those are the criteria for addition to US core
