I have logged and we will correct in the April release. Can I ask if you identified the issues through some sort of FHIR validation mechanism?

No, unfortunately not. I have been maintaining a BCP47 CodeSystem (extract only), and I converted all the VS includes into a CSV for addition into my CodeSystem, and Ontoserver complained when I tried to create the CodeSystem with the extra codes

Hmm, http://hl7.org/fhir/STU3/codesystem.html#invs has
csd-1: Within a code system definition, all the codes SHALL be unique (expression : (concept.code | descendants().concept.code).isDistinct())
so I would have expected $validate to pick that up
