I had a look at Gello a few years back but it only had one (commercial) implementation and seemed to lack momentum

I'd say because we haven't gotten that far yet. But bear in mind that I'm no expert in this area, so there could easily be stuff going on that I have no exposure to.

the thing I'm finding intriguing is, for the first time (that I've seen at least) there is a serious attempt to harmonise quality assurance logic with clinical DS

there's no real infrastructure to support CDSS (because it is so speculative, and the source data is missing for most part) - but there is plenty invested in QA (clinical indicators and audit)...

I'm now at a point where I have a CQL -> js transpiler very near finished. The missing components are a "runtime" (pretty simple just to implement the CQL functions), a FHIR client stub (hopefully less than a 100 lines on top of a REST module), oh and a truck load of test cases
