Just noticed in the logs that with granular scopes, some FHIR API requests were generating 500s instead of providing a helpful error like "your granular scopes don't allow you to make that request". I've pushed a bugfix.

(Since Bas alerted me to errors earlier, I'm keeping a logs -f window open from the demo stack -- this is very helpful for situational awareness at the connectathon.)
