I have a question about Smart scopes and custom operations - how to make it work together?

We haven't defined a syntax for this. I think we'll want to do so; there's been a bit of discussion on the SMART mailing list

The question is whether to do this as a sort of spot-fix or as part of a larger overhaul of our scope language.

I'm still a fan of a format for operations that I've seen suggested by others - user/ValueSet.$expand

I like this just fine! Should we write this up in the current drafted the SMART spec on GitHub?

One complication we'd want to cover was what should happen if a client doesn't know to ask for the scope as it didn't exist before.
I suggested that this knowledge might need to go into the auth server processing the scope requests, and just add them on for defined clients.
