@Ajay Kanduru was able to hack the HAPI SMART on FHIR launch page so that a SMART context can be provided for an R4 server. We've put it up at https://lforms-smart-fhir.nlm.nih.gov/. (We can't guarantee how long that will be up, but you are welcome to use it while it is.)
