GF#13609 proposes that we add the smart capability statement extensions to the FHIR spec- but that's actually a problem for me, because I don't have a framework for declaring extensions that aren't actually defined with a FHIR canonical URL.

will the extension be properly rooted when the ballot completes? If so, lets do it then. Why does it need to be 'in the FHIR spec'?

no, for legacy reasons. I balloted about changing them to be properly rooted, but the decision was not to change them (I was not surprised, but wanted to at least have it discussed)
