Can I confirm that the value of an element with a datatype of 'code' and a binding strength of 'required' must come from the english valueSet. If this is an international site (in this case chinese) it is up to the UI to convert that into the relevant translated display

we've not said that. Most required bindings are associated with 'code' type, and the display is not in the instance for any language.

if the required binding is on a Coding or a CodeableConcept, then the code is required, but any display is allowed.

what I meant was that the instance values for, say, MedicationOrder.status must be one of the english strings: active | on-hold | completed | entered-in-error | stopped | draft - it cannot be the chinese translation. A chinese application needs to understand those strings - however they may present them to the user in a UI...
