Currently hapi ignores contained ValueSet resources in Questionnaire. If you POST a Questionnaire resource to the current hapi server, it will ignore contained resources and not persist them. They are already missing in the response to the POST request itself.
Other contained elements (e.g. contained Practicioner in Condition are persisted).

Will try to look at the issue, but i'm not sure when i will have time for this.
@James Agnew is this already known?
