If an element has no value, but has an extension, it's not serialized (at least to xml, haven't tried json). It should be, or the technique of using a null-flavor extension to provide a mandatory element without a value isn't possible. See EmptyElementWithExtensionTest.java for an ignored test demonstrating the issue.
