How should the following two SMART scopes be understood? Are they any different?
user/Observation.read where the user is Patient/123
patient/Observation.read where the patient is Patient/123

In the "user" case, the individual may have access to more records than just their own (e.g., their children) -- so in general, that scope may be broader.
