It's been pointed out that there are 2 religion extensions currently proposed - https://www.hl7.org/fhir/extension-us-core-religion.html and http://build.fhir.org/extension-patient-religion.html I appreciate that one is US specific and the other not, but beyond the obvious realm-specific nature (When in the US... ) - any other implementer advice? Is this something that could/should be harmonized?

It ought to be harmonized. Ideally US core would use the generic one, but in their profile they'd tighten the binding to the US value set.

Alternatively they can create an extension thats a profile on the base extension - it would be a different URL to reference it, but the same URL in the instance.

Can you expand on that last comment? I had thought that url in the instance was to the 'canonical' url of the extension definition (Structure Definition) and was bound to a single ValueSet...
