Are Race and Ethnicity not discrete FHIR fields? Are they only available through extensions?

Some regions do not allow for the capture of that information. Those that do (or require it) do so through extensions

Also, different jurisdictions typically capture vastly different information. The value sets are typically driven by historic patterns of disadvantage. If you're interested in clinical information about ancestry, that data would typically live in Observations rather than as extensions in Patient, which is primarily about demographics.
