What FHIR resource(s) have people been using to capture the notes a Provider takes when rounding? In our world of Dialysis, the nephrologist is required to round once a week and produce a Basic Note, and then once a month produce a more detailed Comprehensive Note.

Does that mean Observation and ClinicalImpression both can be used for Clinical assessment?

I think that ClinicalImpression main intent was to record clinical assessments ( the "A" in SOAP ). However, there has been a great deal of hand wringing over this resource and it still remains in draft status. ( see the STU notes )
