Can someone help me out with the creation of a new fhir profile for Questionnaire based on the SDC profiles for Questionnaire and Questionnaire Response? I downloaded Forge, see the resources when a select New Profile, but don't see a possibility of selecting the SDC profile for Questionnaire.

The new SDC profiles use FHIR R4 and Forge only supports R3. (It'll support R4 sometime in the spring.)

You could try using Trifolia, you could try creating your profiles by hand or you could try to create an R3-based version of SDC. (The only official releases are the R2 one and the draft R4 one.)

Are you wanting to create a new questionnaire, or profile for a set of questionnaires?
