Topic: mark Encounter as Confidential

I've been asked to mark certain Encounters as confidential, but I don't see a well-defined place to put this in stu3 or r4 Encounter. I'd prefer not to add our own extension in for this.
Would adding an additional type to encounter.type -- pointing to our own valueset URI for this be appropriate, or are there better ways to do this?
