1.I took the identity of the patient which was issued by the government in the Patient Resource(Identifier). Where is the date and place of issue? Or any other resource may indicate?
2. What 's Resource indicate nationality and religion ?

#2 the specification defines extensions for religion, nationality, and citizenship

Are there any rules that create extensions? Does Fhir server accept it when validate ? The url of extension is a mandatory attribute. Are there rules here?

Extensions are defined with the StructureDefinition resource as profiles of Extension. If the structure definition is known to the FHIR server, the server can use the extension in validation, the same as any other profile.
The URL of the extension is its unique identifier. It has to be a valid URL and be unique.

If there's a "standard" extension (defined by HL7, IHE, your national organization or anyone else that likely has significant adoption, you should use that existing extension (which means using the existing URL). However, if you define your own extension for something new, you would define the URL

You identify that profile by its own URI.
If you create your own profile, give it a URI from a name space which you control or one in which you can assign URIs.
Your FHIR server will know about your extension the same way that it knows about any other profile that you define.

Thanks everyone :D .
In my country, there was very few integrated organization hl7.I had a lot of difficulties when I was self-manual hl7 messages to match my health system. Of course still comply with hl7 fhir standards.Then I sent Guidelines to partner. My Partner must follow this guidelines.
Do you do that with your system? Let's share to everyone :D What is the final result you achieved ? To connect many system or announced to the world that your system has integrated hl7

It depends on the environment you're in. In some places, there's large communities that come together around a specification. In others, a regulator dictates. But there certainly are environments like yours where one partner implements a FHIR interfaces and then works with partners to have them adopt the same specification.
