If you extend an extension (I'm needing to add 'datum' to geocode - http://hl7.org/fhir/extension-geolocation.html - does the canonical url change? I'm assuming it does, but would like to confirm...

That would be a modifier extension, and I would expect break many implementations using a different coordinate system.

Yup, a different extension for your datum geocode. This would also be required for conversion with the boundary extension in location, and the near search param and contains.
