Given that in many places it's not ok to record ethnicity, how is this normally supplied for a lab test, when it may affect the result? In Patient using us-core-ethnicity? In the ProcedureRequest using a supportingInfo Observation?

SupportingInfo would be the best option. If you're passing it for clinical purposes, then you'd want a clinical value. The data on Patient are captured for administrative purposes and may have little clinical/genetic relevence - though they could be a fall-back if you have nothing better. SupportingInfo also lets you explicitly call out the information as potentially relevant. Demographics on the patient are just "there".
