Under STU3 and R4, you'd have to use extensions to correlate images referenced in ImagingStudy to Binary resources. The images wouldn't be attachments to the ImagingStudy. You could use multiple Media resources instead of ImagingStudy, or in conjunction with ImagingStudy, but I don't think that gets you there either. We purposely designed ImagingStudy for use with a DICOM archive.

Depending on what you are trying to accomplish, you might want to connect to the PACS system instead of Epic/The EHR. EHRs and RIS systems hold documentation, and I think Epic has enough data to say, hydrate an ImagingStudy, but the PACS will hold the actual binary DICOM file.

Not available. The Binary only returns clinical notes from what I can see. ImagingStudy is not out yet.

Also, @Nick Hatt was talking about retrieving the (lowercase) binary image content from the PACS, not Binary resources. By the way, there is no requirement that the EMR populate the ImagingStudy resource or be responsible for storing it. Both of those could theoretically be done by the PACS.
