FYI - I'm working on an exercise for my testing session where I hope to engage some attendees with developing TestScripts for an upcoming FHIR Connectathon test track and the Subscriptions Track is one possibility.

@Jens Villadsen Unable to attend DevDays this year but very interested in hearing about the discussions on subscription. I am f.ex. interested in how/if FHIR subscriptions can be implemented using messaging systems like Apache Kafka or RabbitMQ. We are using Kafka with FHIR, but in a non-standard way. I know IBM are doing similar stuff when Watson pub/subs FHIR resources.

in FHIR terms this is like having a way to write, say name.family="Marks" (the R3 syntax)and have it be mapped to name.family=["Marks"] in R2

But do you need to put support for any of those (transport protocols) into FHIR? I've used ActiveMQ (as a queue) using Http Request and Response objects which contain FHIR Bundles.
