Just to check - this is the standard extension to use if information is declined to be given: http://hl7.org/fhir/extension-iso21090-nullflavor.html . (I have this memory that there is another, but can't readily find it)...

The preferred extension is http://hl7.org/fhir/extension-data-absent-reason.html
