We have a couple of searches we would like to add to our Medication resource to provide additional capabilities beyond the standard lookup/search. For example to find a medication related to another, ie give me the brands of this particular med. This is backed by our own drug model (not a true terminology server so don’t have valueset or $closure capabilities, but we have our own logic for this). Is there any precedence of others doing a similar thing or are we forging new ground and ok to proceed, with documentation for consumers on how to use the searches?

there search parameters are for convenience. You can define your own search parameters. If you define them formally using the SearchParameter resource, and reference that in your capability statement, that's even better, since introspection will work for them too.
