Topic: best resource for storing information about ADL

We gather information in one of our assessments about how well the patient says they are able to perform activities of daily living (ADL), such as bathing, dressing, etc. is there a resource to store that information aside from the generic questionairre? maybe observation?

A questionnaire could be appropriate to create a form to collect the data, then convert the response into the individual observation resources.
