If you're converting from v2 to FHIR message, you can generate GUIDs as your ids and they only establish linkage within the context of the message. If you're trying to turn v2 into FHIR REST, then the question is how you determine that a medication being referenced in v2 is an identifiable instance in FHIR terms. I expect most of the time, you'll end up converting to 'contained'.
