Is the way the process works between not and R4 that what we see on build.fhir.org is locked down until the R4 release? (with the only changes expected to be items that are rejected during voting?)

not quite so simple. build.fhir.org is the trunk. Normative content is locked down and only changes now are ballot related, or editorial corrections. Trial Use content is much more subject to change.
