If we want to stick with the recommended V3 ActEncounterCode Value Set what should we do when the Encounter class is truly unknown?

It’s a FHIR question. The specification for Encounter.class says to use the V3 ActEncounterClass.

The binding is Extensible. "If there is no applicable concept in value set (based on human review), an alternate concept (either system/code pair, or text) may be used instead. " I think a good candidate is Data-Absent-Reason value set: http://hl7.org/fhir/valueset-data-absent-reason.html
