Note: I would like to use it for confirmance testing

The FHIR test servers also support the US Core Profiles if you specify the profile in meta.

https://inferno.healthit.gov/reference-server/r4 is the root endpoint. You cannot access it without token. https://inferno.healthit.gov/reference-server/r4/metadata is public accessible

you can set the following header to get access: Authorization: Bearer SAMPLE_ACCESS_TOKEN.cGF0aWVudC8qLio=
