Can we expect a stand alone Goal for a patient, or will it always will be accompanied with a CarePlan?

Stand-alone goals are absolutely possible. Whether you can expect them or not depends on who your implementation partners are :)

@Lloyd McKenzie we are trying to implement FHIR Server so as to comply with the CMS 9115-F. So in this regards can we still expect stand alone goals? If so then any reason why the goal identifier isnt marked as MUST Support?

I'm not sure what CMS's rules are around Goals. Having them as stand-alone doesn't mean that business identifiers are essential. They would have a RESTful id. Business identifiers would primarily be useful if there was an expectation for goals to be managed across systems, and I'm not sure that would be widely supported.

Cerner servers also support standalone goals: http://fhir.cerner.com/millennium/overview/#supported-resources-between-versions (both DSTU 2 and R4 implementations)

They're literally allowed to be anything :) EG: I want to lose weight.

They may or may not be tied to a larger CarePlan. EG: Manage diabetes.

@Jenni Syed In regards to the example shared isnt that more of a personal goal. How are we planning to receive this information as part of EMRs?
Are we saying the EMRs even capture this? Who generally will be updating this? Will that person be the primary care provider for the patient?

Based on what I understand, these goals can be patient stated, provider stated (with or without a larger care plan - eg: your provider may say "you need to lose weight" without it being part of a larger plan), or something a family member has stated

I assume what Michele is pointing out is that we build in the capabilities to allow many different workflows. How systems use it may vary :)
