The profilesin the US-Core R4 inherit from the base specification where the meta.profile tag is optional. In the examples, the profile tag is always populated. Is it explicitly mentioned in the Implementation Guide? or is it an understanding that the profile tag is needed in the instance?

The profile is not required. It is used in these example so that the built in validation in the publication tooling knows which profile to validate against.
