Is there a std doc I can point ppl to wrt how a given fhir uri should look wrt versions e.g. should version be in the header of the message (as I believe) vs in the uri etc. ? Atm I am looking at a heinz 57 approach to the subject & would like to be able to say "this is the std".

General summary is that there's no requirement to declare a version other than in the server's CapabilityStatement. Requesting a specific version might be by URL (if a server has different version-specific endpoints) or via the Accept header (if a server supports that).
