We are planning to get R4 in Simplifier by the end of January. There are quite some sub systems that need to be updated for that. Some parts will be ready later.

Forge for FHIR R4 requires quite a bit of re-work, to handle breaking changes in behavior, especially constraints on element types and choice types. These changes require re-design of some aspects of the UI. We aim to publish a stable Forge R4 release before the next WGM in May 2019.
