@Lloyd McKenzie & co, any advance on this Stack Overflow Post on providing examples of searching against an extension?
There are example instances of SearchParameter and examples of Conformance/CapabilityStatement referencing SearchParameter, but none that show SearchParameter searching against an extension that I'm aware of in either the HL7 core spec or in any of the IGs we publish - it just hasn't come up. (I haven't had a need yet in any of my non-HL7 IGs yet either.) Feel free to submit a change request advocating for such an example to be included in the spec.

Looking at this (boolean) extension on the Condition Resource...

What should appear in the query string - I can see two possibilities
a. GET [base]/Condition?extension= http://hl7.org.nz/fhir/StructureDefinition/long-term-condition|True
b. GET [base]/Condition?long-term-condition=true

It is (b) - the server has to add new custom search parameters corresponding to the extension
