I have a field for review Instructions where the doctor writes something like -
"meet after 10 days" or "revisit after 15 days" or "revisit if not better"

What's the context in which they're making that statement? I.e. what are the other data elements you're capturing at the same time?

sound like part of the P in SOAP so I would say CarePlan. (or a clinical note that is referenced by it)
