Good afternoon,
I've been searching for a way to define mime-type and size restrictions on a questionnaire question of type "attachment", right on question definition without having to define a StructureDefinition.
Is there any standardised way of achieving this outcome?

Yup. Look at http://hl7.org/fhir/extension-mimetype.html and http://hl7.org/fhir/extension-maxsize.html
