And now I've written this into the specification. See http://build.fhir.org/http.html#version-parameter for the substantive change part, http://build.fhir.org/versioning.html for the documentation, and there are references to this stuff from about 10 different places in the spec (including from the front page)

@Grahame Grieve The version specific profile address my ballot comment and therefore I withdraw. The addition of the payload version to the html is useful for exchanges using FHIR html.
