We do diagnose clarification in Psychiatry with CarePlan. Also Relatives (parents) and Teachers fill in QuestionaireResponse (different questionnaires to Patient, Parents and Teachers). Now should we have one CarePlan for Patient and another for the RelatedPerson ... OR in what better way put the Questionnaire as a task for the Parents and Teachers inside the CarePlan of the Patient

One Care plan with distinct activities, each with their own performers would work. You can also use Task to manage the individual requests.
