I need to represent some basic data about radiotherapy (substantially the target site and the cumulative dose) , with similar info if a boost is given. (see figure below)

I was thinking to use a procedure for the boost linked to the radiotherapy procedure by the partOf element;
describe with an observation the cumulative dose (using the dicom code 113725 "Dose (RP) Total");
and define an extension for the procedure (relatedInfo) to link the procedures with the cumulative dose observation.

You may want to check with @Abderrazek Boufahja and the Imaging work group who are currently working on modelling raditation dose.

Hi @Giorgio Cangioli and @Elliot Silver , we worked within II WG on the IG "Radiation Dose Summary for Diagnostic Procedures": https://build.fhir.org/ig/HL7/fhir-radiation-dose-summary-ig/. The name is long in order to be explicit, and exclude the radiotherapy procedures, which is analysed by another group from Codex: Radiation Therapy Treatment Data for Cancer: https://confluence.hl7.org/display/COD/Radiation+Therapy+Treatment+Data+for+Cancer.
We modelized the Radiation Summary for CT, X-Ray and NM using Observation profiles (one per use case): https://build.fhir.org/ig/HL7/fhir-radiation-dose-summary-ig/archi.html#profiles-relationship . I think this can bring you ideas if you want to use the same modelization technique. We have not used procedure, as the matter for the Radiation Dose Summary IG is about reporting, and not about following the patient treatment; that is why the Codex team was more about using Procedure than Observation resource (maybe things had changed, since our last discussion with codex team).
