I wonder if there is a guide on how to breakdown questionnaireResponse answers to Observations. couldn't find any around.

Take a look at the SDC implementation guide - specifically the page on 'extraction'. It talks about how to design a Questionnaire whose QuestionnaireResponses can be automatically turned into other resources, including Observations. http://build.fhir.org/ig/HL7/sdc/extraction.html
