I would like to ask, if there some reason why cardinality of identifier-element in QuestionnaireResponse-resource is 0..1 instead of 0.. *. In many other resources,identifier's cardinality is 0.. *, e.g. in Questionnaire.

Primarily because we haven't heard of use-cases for (or most systems supporting) multiple identifiers for a single response. Feel free to submit a change request if you think we got it wrong :) (Use the 'propose a change' link on any page in the spec.)
