The 'attachment' QuestionnaireItemType (http://hl7.org/fhir/codesystem-item-type.html) value is restricted to items of type 'question' if I understand correctly (whether it's captured at validation or not I'm not sure). However it seems to me that one could also want 'display' items to be rendered as e.g pictures/icons (in the form of binary attachments)? Would it be useful to describe both 'text' (which also seems unique to 'question') and 'attachment' as applicable to 'display' as well, in this valueSet?

No. If you wanted a display item to include graphics, you'd use the SDC extension to convey your text as XHTML
