Topic: Questionnaire Sources
Matthew Mosier (Aug 22 2019 at 19:27):
I am new to the FHIR scene and have heard that there are thousands of FHIR questionnaires out there. Yet, I can't seem to find any single source of these questionnaires. Is there an api I can call (or even a .zip to download) that I can populate my web page with?
Lloyd McKenzie (Aug 22 2019 at 19:32):
It's likely that there are thousands of FHIR questionnaires out there, but if so, they're primarily in private repositories for particular organizations - not necessarily intended for general use. What types of questionnaires are you looking for? What are you hoping to accomplish with them?
Matthew Mosier (Aug 22 2019 at 19:53):
I'm hoping to create a small web app that lets people create, view, and upload questionnaires to an R4 server. I would like to have it populated with a large number of preexisting forms. Looking for a wide range, but pregnancy related forms should be included.

Best place to solicit is probably the #questionnaire stream. @Brian Postlethwaite @Paul Lynch @Grahame Grieve
Paul Lynch (Aug 22 2019 at 20:08):
We have a FHIR server at https://lforms-fhir.nlm.nih.gov/baseR4/Questionnaire which has LOINC panels as (unofficial) Questionnaires, plus some made up Questionnaires.
Noemi Deppenwiese (Aug 23 2019 at 08:27):
https://medical-data-models.org/?lang=en also has a "download as FHIR" option.
Daniel Vreeman (Sep 14 2019 at 18:13):
We've been doing some pilot work to create a FHIR Questionnaire representation of LOINC panels and forms based on the Argonaut profile. They're available for testing, but not yet part of the official release/distribution. With a loinc.org account (auth required), you can access them here https://fhir-dev.loinc.org/Questionnaire/. Just a warning that this is a temp server. But one you can try out this weekend.
