https://www.hl7.org/fhir/valueset-procedure-outcome.xml.html - this defined three SNOMED codes as Procedure outcome codes. But there is no explanation of these codes available. I can see that this ValueSet is Example, that means, we have to come up or decide our own codes, is it so?

Those codes correspond to "successful", "unsuccessful" and "partially successful". But they could well include things like "restored full function", "patient deceased", "moderate improvement of pain" or all sorts of other things - depending on the type of procedure.
