I am confused between Condition and Observation

Condition is for specific diagnoses. Observation would let you capture ad-hoc history descriptions.

So in the case of Observation , which category should i use. I can not see any relevant other than "social-history".
Or without category it will be fine to go?

No category is allowed. My take is that social-history would be appropriate, but I'd defer to folks from the orders & observations work group. @Eric Haas ?
