I need to do a search for all Patients that are not referenced in Appointments, on a HAPI server
Any suggestions on how to do this?

You might ask on #hapi. I don't think there's a standard way to do this. It seems like it would have to be a negation of reverse includes and there's no standard for that at the moment.
