Hi everyone, what API and fields track a patient's history with a practitioner? Specifically: when did the practitioner become the patient's PCP, previews PCP's, Reason for changing PCP, and time/date of the change.

CareTeam (and the history of it) together with Provenance should be able to capture all of those things
